Thank you for your interest in Baptist Health System's HeartAware heart disease risk assessment! You have taken the first step to learning more about your heart health. In only seven minutes, this innovative tool can help identify your risk for heart disease, the nation's #1 cause of death.

At the end of the online assessment, you will view a report about your heart disease risks. If you are deemed "at risk" for heart disease, you will have the option to schedule a free follow-up appointment with a cardiac rehab nurse at a Baptist Health System hospital for a more detailed report and additional screenings. These screenings include blood pressure measurement, a finger-stick blood test to measure cholesterol and glucose, weight and body mass index and other tests as indicated. The results can help identify the need for early intervention -- even if no symptoms have been experienced.
